It is mandatory for all students to fill paf form as it will help us to monitor the attendance of our students and will also help in finding out any errors in attendance.

– Your name and address

– Your date of birth

– The child’s name and date of birth

– The child’s gender, if known

– Signature of person completing the online registration form

  1. Please print, complete and submit a Registration Slip for each child whose name appears in this database that you wish to register. You must also print the letter from the school or school district requesting that you submit this form. (A copy of this letter may be obtained from the school or school district.)
